A program orientation meeting will need to be scheduled. At the time of orientation, the potential clients are provided an overview of the House of Mercy services and introduced to the facility so that the individual can decide whether the program will meet her needs. Requests for outpatient services or other programming can be made by phoning 515-643-6500. At the time of the call a program orientation interview meeting may be scheduled.
Referring workers may contact Leanna Brady, adolescent program supervisor, by phone (515-643-6500), letter, or e-mail Leanna. A complete assessment is a critical part of the referral process. This process begins when a placing worker makes inquiry as to the prospective student's appropriateness for the In Touch with Teens program. Placing workers consult with the program supervisor and are asked to forward pertinent documentation regarding the youth. Existing documentation required includes:
Utilizing all referral materials as well as verbal reports of placing worker, the supervisor will determine the prospective student's eligibility and appropriateness for placement at House of Mercy.
Guardians are to be present at admission if releases have not been signed prior to admission.
